* If there are any questions, please call the school office at 570-2220.
Documents needed:
1. New students to WCPSS need to bring
birth certificate, proof of residency, immunizations, adult
picture ID. (If kindergartener, they also need completed
NC Kindergarten Health Assessment.)
2. Wake County student with change of address need to bring proof of residency
with address.
* Proof of residency
can be gas bill, utility bill, lease agreement, purchase
agreement with less than 45 days until closing.

Registration
for Rising Kindergarteners |
In an effort to place siblings on the same track, please come to the school office
and complete our registration packet right away! Your child does not have to
be present to be registered.
Documents needed:
* Proof of residence (Examples include a current electric, city gas
or city water bill, or a signed lease agreement/ closing statement in the name
of the parent(s)/ court-appointed guardian with a closing date within 45 days.
(Telephone, cable television bills, and driver’s licenses do not qualify)
* A certified copy of the child’s birth certificate (This
can be obtained from the county or state health dept. where the child was born).
* Parent/Guardian Picture ID
* Kindergarten Health Assessment form: North Carolina law requires
that this form be completed before a child can enter kindergarten. The health
assessment must be completed no more than 12 months prior to beginning kindergarten.
* An updated immunization record. Medical evidence of the following
must be provided:
* DTP: 5 doses. If the
4th dose was given on or after 4th birthday, the 5th dose
is not required.
* Polio: 4 doses. If the 3rd dose was given on or after 4th birthday, the
4th dose is not required.
* MMR: 2 doses received at least 30 days apart
* Hib: 1 dose. Hib Not required if the child is 5 years or older
* Hep B: 3 doses
* Varicella: 1 dose
